Here’s a fairly lengthy discussion on the topic of contact forms in Shopify: http://forums.shopify.com/categories/1/posts/1363 To summarize…Shopify doesn’t have contact forms. Here’s a list of some of the paid third-party form services you could use to put one on your site fairly easily though: www.wufoo.com Personally when I was researching this, I thought these services were a little overpriced. I ended up going with ZOHO Creator (www.zohocreator.com) which is an online application/database builder that lets you build your own contact form with no programming knowledge at all (online “drag and drop” builder). It’s free up to 5 “apps”, more than enough if you just want 1 form (but they do have a very small zoho logo at the bottom of the form) See mine here (http://www.shopcora.com/pages/contact) I would assume the paid contact form services have better reliability/support but I’m not completely sure (so far so good with Zoho for me). I’ll probably move to a paid service once we get more traffic. You could also purchase/download a contact form script to host on your own and run off another server. Kaushal ---

An easy way for beginners is to use the form generator on icebrrg.com. They are free unless you expect a lot of entries. It is turnkey and will give you a code snippet to paste into your page on shopify, don’t forget to surround the code with
